Diane Slotemaker, OD

Dr. Slotemaker has a bachelor of science degree from Calvin University in Grand Rapids, Michigan and a doctor of optometry degree from Illinois College of Optometry in Chicago. She has completed externships at Sidney Hillman Health Center in Chicago as well as at the Illinois Masonic Hospital and has been practicing optometry since 1997.

Her specialty is primary eye care with experience in contact lenses and urgent care. When she can, Dr. Slotemaker enjoys volunteering internationally with OneSight. So far OneSight has taken her to China, Peru, Colombia, Guatemala and Zambia. Here in Illinois she keeps busy with her family. She’s married with four children.
